The Candyman Summer Rock Camp is committed to conducting its programs and activities in the safest manner possible. Participants and parents/guardians registering their child/ward must recognize that there is an inherent risk of injury when choosing to participate. We continually strive to reduce such risks and insist that all participants follow rules, code of conduct, and instructions.

 

The Candyman does not carry medical insurance for injuries sustained by participants as the cost of such insurance would make program fees prohibitive. Therefore, each person registering themselves or a family member for Summer Rock Camp should review their own health insurance policy for coverage. The absence of health insurance coverage does not make The Candyman Strings & Things or The Candyman Summer Rock Camp responsible for payment of medical expenses. The agency providing liability coverage for The Candyman REQUIRES you to sign this Waiver and Release. Your cooperation is greatly appreciated. Please read this form carefully and be aware that in registering your minor child/ward for participation in The Candyman Summer Rock Camp, you will be waiving and releasing all claims for injuries your child/ward might sustain arising out of these programs.

 

Release of Claims 

 

As a participant in The Candyman Summer Rock Camp, I agree to assume full risk of any injuries, including death, damages or loss regardless of severity, which my child/ward may sustain as a result of participating in any and all activities connected with or associated with the camp. I agree to waive and relinquish all claims I or my child/ward may have as a result of participating in the Camp against The Candyman Strings & Things, The Candyman Summer Rock Camp and its owners, agents, volunteers and employees. I do hereby fully release and discharge The Candyman Strings & Things, The Candyman Summer Rock Camp and their respective owners, agents, volunteers and employees from any and all claims from injuries, including death, damage or loss which I or my child/ward may have or which may accrue to me on account of my participation.

 

I further agree to indemnify and hold harmless and defend The Candyman Strings & Things, The Candyman Summer Rock Camp and their respective owners, agents, volunteers and employees from any and all claims from injuries, including death, damages and losses sustained by me or my child/ward or arising out of, connected with, or in any way associated with the activities of the program. I do hereby fully release and discharge The Candyman Strings & Things, The Candyman Summer Rock Camp and their respective owners, agents, volunteers and employees from any and all claims resulting from loss or damage of personal property.

Agreement

By enrolling a student in Summer Rock Camp, it is binding that this agreement is understandable and agreeable by the Signatore. A digital signature, included on the online registration, executes this agreement.
