By enrolling my child as a participant of The Candyman Summer Rock Camp, I understand that I am granting permission for my child to participate in The Candyman Summer Rock Camp field trips and I acknowledge and agree to The Candyman Summer Rock Camp Field Trip Permission policy.

I understand my child will be transported by bus, auto, limo, cab, Lyft or Uber, to various locations and will return to The Candyman prior to parent pick up.

Field trips will include transportation to Hutton Broadcasting on the second Friday of camp for the radio station interview. Additional field trips will be at the discretion of the Instructor and/or Camp Director and may include walks to nearby parks, restaurants, convenience stores, etc.

 

RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AND PARENTAL CONSENT AGREEMENT:

I hereby release and discharge The Candyman Strings & Things, its agents, employees, staff members, and owners from any claims, responsibilities or liabilities for injuries or harm incurred as a result of my participation and/or my child’s participation as a participant in camp activities.

I authorize The Candyman Strings & Things, its agents, employees, staff members, and owners to take whatever action is necessary, in their best judgment, in an emergency and I hereby release discharge to The Candyman Strings & Things its agents, employees, staff members, and owners from any responsibility or liability related thereto.

Agreement

By enrolling a student in Summer Rock Camp, it is binding that these policies are understandable and agreeable by the Signatore. A digital signature, included on the online registration, executes this agreement.
